The Practice

Case Management

Case Managers are exceptionally experienced individuals uniquely able to relate to clients, form a bond of trust, and lead the mental health treatment effort. Case Management acts as the central hub, guiding and coordinating Turnbridge services around the needs of each client. Case Managers come to know and understand each client, and identify needs, strengths and opportunities. They also become strong guides, role models and assistants that clients rely upon to help learn, grow, and navigate the mental health and addiction treatment process until they’re able to find their own way.

Case Managers are responsible for the coordination of care, directing mental health treatment programs, organizing recreation and wellness activities, teaching financial management, fostering life skills development, availing educational and vocational resources, and acting as the primary point of contact for family members.
